saya agak bimbang dengan borang anda. pemohon diminta memberikan banyak maklumat peribadi
tetapi:
1) anda tidak menyatakan nama syarikat anda
2) tiada nombor SSM
3) tiada lokasi pejabat.
tanpa maklumat diatas, sukar utk pemohon memastikan maklumat mereka tidak disalahgunakan utk tujuan lain termasuk tujuan jenayah.
